高工作壓力的環境降低工作滿意度,護理主管應藉傾聽,了解基層護理人員實際經驗的壓力感受,讓其感受到被支持與關懷,進而留任。本研究係以質性訪談方式,針對12個不同單位因工作壓力尋求單位護理長協助之28位護理師進行輔導會談,會談結果以內容分析法進行資料分析,並以真實性、可確認性、以及可轉移性進行研究嚴謹度檢視。結果發現基層護理師內心工作壓力為一、「人際關係緊張」:不被瞭解與信任及遭受背叛;二、「專業認同不足」:角色貶抑及看不到成就感;三、「無力感」:蠟燭兩頭燒、失去信心及無法面對自己的情緒。單位主管應刻意營造同仁間相互了解機會及藉合作強化情感;加強平日工作的成就感及加入專業團體營造自身護理專業認知;並同感基層護理人員在面臨異常事件發生時之心理衝擊;辦理相關教育及成立固定支持團體,協助癌症病房護理人員走出面對不斷死別所引發之情緒,以加強護理人員留任。
Background: Employees working under high stress decrease the degree of satisfaction for their jobs. Nursing managers need to understand the experience of actual stresses of staff nurses by listening attentively to them. By doing so, nurses can really feel the support and consideration and then are willing to retain their jobs. This is a qualitative study, and 28 stress-facing staff nurses from 12 different wards are interviewed by their head nurses. The interview results are analyzed by content. The rigor of the analysis is verified by authenticity, confirmability and transferability. The results show that the actual stresses of staff nurses can be classified into 3 dimensions. 1. 「tensions from interpersonal relationship」: this type of nurses think they can't be understood, not be trusted and are betrayed. 2.「insufficient identification about their own profession」: this type of nurses always devalue their role and have no sense of achievement. 3.「the sense of powerlessness」: this type of nurses feel exhausted, lose confidences and unable to handle their own emotion.Conclusions/Implications for practice: From the analytical results, department managers should take the following measures to reduce the stress of staff nurses to increase nurse residency: 1. Create the opportunity intentionally for coworkers to understand each other and reinforce the mutual emotions through cooperation. 2. Increase the sense of achievement from causal works and enhance the identification of their own profession by joining the specialty groups. 3. Have likewise feeling with staff nurses who face the inner-heart impulse for handling accident events. 4. Held the related training and form the constant supporting organization in order to help the nurses at cancer ward to smooth the emotion from facing death frequently.
