摘要

顳頜關節之腱鞘囊腫是一罕見的部位，但是對於一耳前部位的腫瘤是一個重要的鑑別診斷之一。西元一九七七年，由Heydt首先有第一個病例報告。文獻查證顯示，曾經被報告過的病例數目，不超過三十個。也由於它的罕見，通常在手術前，正確的診斷難以建立。我們報告一位六十二歲的女性病人，主訴是：位於右耳前方有一腫塊性的病兆已經三年了，該病患並無創傷之病史。理學檢查顯示，右耳前方有一稍硬的腫塊，大約有一點五公分乘以一公分大小。電腦斷層顯示，有一界限明確的節結，靠近右側的腮腺。初步的臆斷為：右腮腺腫瘤。在手術中意外的發現，原來是一個顳頜關節之腱鞘囊腫，非常靠近顏面神經。組織病理切片檢查，確立診斷為顳頜關節之腱鞘囊腫。病人術後追蹤三個月，狀況良好沒有復發之現象。我們報告這個病例回顧相關文獻。

A ganglion cyst of the temporomandibular joint (TMJ) is a rare location but it is important in the differential diagnosis of a preauricular swelling. Heydt first described a TMJ ganglion cyst as a preauricular mass in 1977. About less than 30 cases have been reported in medical literature. This lesion is very difficult to diagnose correctly before surgical exploration, because of its rare occurrence. A case of 62-year-old woman with one mass lesion in the right preauricular region is presented. The CT scans of the lesion showed presence of a well-defined nodular lesion, about 15×10 mm in dimension with homogenous density. Under the impression of right parotid gland tumor, she received the surgery. An incidental finding was noted during the operation. The TMJ ganglion cyst was diagnosed. After 3 months follow-up, no recurrence of the TMJ ganglion cyst was noted. We discuss this case and review the past reports.
