Reconstruction of Female Poland Syndrome with De-epithelialized Pedicled Transverse Rectus Abdominis Myocutaneous Flap

利用去上皮化的橫式腹肌莖肌皮瓣來重建女性波蘭氏症候群

摘要


波蘭氏症候群是一種罕見的先天異常,它的特徵是乳房及胸壁的畸形,造成男性或女性患者外觀上極大的影響。我們報告一位四十歲的女性患者,來診時呈現胸壁凹陷及單側乳房發育不全,經評估後,我們經由腋下小型切口創造一個皮下袋並轉移去上皮化的橫式腹直肌莖肌皮瓣來重建患者乳房。使用這種方法,術後胸口沒有明顯的疤痕組織,而且獲得令人滿意的外觀,除此之外,患者可以額外得到腹部整形的好處。我們報告這個有趣的案例並整理相關的文獻報告。

Poland's syndrome is a rare congenital anomaly characterized by deformities of the breast and chest wall that can be highly disfiguring in both men and women. We report a 41-year-old female presented with significant unilateral hypoplasia of right breast and chest wall developmental scarcity. We transferred a de-epithelialized pedicled transverse rectus abdominis myocutaneous (TRAM) flap into a subcutaneous pocket created through small transaxillary incision. Satisfied result was achieved with additional aesthetic effect of the abdominoplasty and no obvious scar was made over the breast region. We presented this interesting case and review the literatures.
