促進高血壓老年人服藥遵從性之家庭護理經驗

摘要

本文藉由照顧一位有高血壓老年患者的家庭，於家訪期間，運用觀察、會談技巧及Friedman的家庭評估模式收集案家的實料，並使用Gorden護理診斷確認健康問題。筆者於照護期間，針對高血壓老年患者服藥遵從問題，與案家建立信任感及治療性人際關係後，運用相關知識的教導、家屬的支持力量、持續性關懷、正向回饋、增強健康信念、並建立服藥規則性等護理措施，改善其服藥行爲。

This article presented the family nursing experience of a hypertensive elderly. Using Friedman's family assessment model, the authors collected the family's data by observation and interviewing. The Gorden's nursing diagnosis was used to identify health problems of the subject. After establishing a mutual understanding and therapeutic relationship with the family, the nursing interventions were. provided, including provision of hypertensive education, encouragement of family supports, continuous caring and positive feedback, reinforcement of family's health beliefs, and establishment of regular medication to improve the medication adherence.
