摘要

本篇個案報告爲一位卵巢癌末期病患之護理經驗。照護期間爲民國92年6月2日至6月7日，經由觀察、會談、並運用Gordon十一項健康功能型態評估進行資料收集，確認個案有自我照顧能力缺失、舒適情況改變－急性疼痛、身體心像紊亂、預期性哀傷等護理問題。藉由筆者的護理措施提供整體性、連續性的評估與照顧，協助病人增加自我照顧能力、提升自主性，減輕疼痛、改善生活品質，接受身體結構功能的改變、維持正向的身體心像，強化夫妻親密關係之建立、與摯愛道別、引導尋求生命的意義及面對死亡。期盼藉此臨床經驗之分享，提供護理人員照顧相關個業時之認識與了解。

This article reported the nursing experience of a patient with end-staged ovarian cancer who received care in June 2003. Data were collected by ”Gordon 11 Functional Health Assessment tool” through observation and counseling. Results showed that the patient had nursing problems including self care deficit, changing of comfort settings, such as acute pain, body image disturbance, and anticipatory grieving. After our nursing implementation with total and continuous evaluation and care, the patient was able to improve her self-care capability, self-autonomy, pain alleviation, quality of life, body function alteration acceptance, positive physical-mind image maintenance, spousal intimacy relationship strengthening, ability to say farewell to her loved ones, and ability to seek the meaning of life and face the reality of death. The authors would like to share this clinical experience with the colleagues to provide better care for this particular group of the patients in the near future.
