摘要

本文爲探討照護一位71歲男性罹忠慢性阻塞性肺病忠者之護理經驗，個案自1998年被診斷慢性阻塞性肺病，近四年來因呼吸喘、呼吸困難反覆入院治療多次，此次係因呼吸道感染引發呼吸喘而住院，除因疾病引發的生理不適外，於長期慢性病求治過程中，亦造成心理上的壓力。筆者於2005年4月22日到2005年5月5日護理期間，運用Gordon十一項功能性健康評估模式進行護理評估，發現個案有呼吸道清除功能失效、活動無耐力及焦慮等護理問題，經由筆者計劃性護理並以自我照護理論爲基礎，給予合適且個別性的護理措施，提昇個案自我照護能力及家屬參與照護，進而達成減少再入院率與提昇良好生活品質。希望藉此護理經驗提供護理同仁照護此類病患時，除了緩解身體上的症狀外，亦能增加病患自我照護能力及家屬參與照護，以達到全人之照護。

This article reported the nursing experiences of a 71 years old men with chronic obstructive pulmonary disease (COPD). The patient has been diagnosed as COPD in 1998. After being hospitalized repeatedly during the past four years, he was hospitalized again due to shortness of breath resulting from respiratory tract infection. During his hospitalization period, we used the Gordon 11 functional health assessment to evaluate his condition. Nursing problems identified included ineffective respiratory tract clearance, poor endurance of activity, and anxiety. Appropriate individualized nursing care was provided to improve his self-care ability and his family participation. The goals were to decrease the re-admission rate and promote his quality of life. This nursing experience emphasized the importance of self-care and family participation and provided nursing staff with a reference of caring similar patient in the future.
