協助失能老人及其外籍看護工的護理經驗

The Nursing Experience of a Frail Elder and Her Foreign Caregiver

摘要


本文在探討一位因重覆感染的失能老人及其外籍看護工之護理經驗。藉由主護護士的角色於(2006年8月2日至18日)照護期間,採Gordon十一項健康功能型態評估,以觀察、會談、傾聽等方式收集主、客觀資料,確定個案的健康問題有:一、呼吸道清除功能失效:二、潛在危險性感染;三、營養狀況改變:少於身體需要;四、照顧者知識缺失。在護理過程中主動關懷,運用同理心與個案及其看護工建立良好的護病關係;協助清除個案呼吸道中的積痰;教導看護工有關導尿管的消毒方法,維持個案每天液體之攝取量以降低潛在性感染的風險;建議調理與提供具有營養價值的飲食;提供中文/印尼文雙語對照之護理衛教單賬,輔以解說照護措施與技術,並回覆示教,以確認正確,減少照顧失當或疏忽情況,提升個案的健康。

This case study described the nursing experiences of a female frail elder with repeated infections. The addition nursing experiences of her foreign caregiver was also described. The nursing care time period was from August 2th to 18th in 2006. The Gordon's eleven-functional-health assessment was applied in the nursing process. Data were collected by observations, interventions and listening. Four major health problems were found, including ineffective airway clearance, risk for infection, altered nutrition (less than body requirements), and the lack of knowledge of this caregiver. Through actively nursing interventions with sympathy, a well nurse-patient relationship was built. In order to eliminate the four health problems, nursing care interventions included promoting sputum drainage to keep respiratory hygiene, teaching the foreign caregiver to perform adequate skills in foley care, and decreasing potential infection risk in maintaining daily liquid amount. In addition, the caregiver was instructed to manage adequate diet for the elder and improve her health. Pamphlet with both Chinese and Indonesian languages regarding nursing care was provided to explain nursing intervention and skills. The caregiver was asked to practice what she had learned repeatedly. Therefore, the risk of inadequate care caused by the caregiver can be reduced. Through the process, the frail elder's health has been improved.
