一位罹患自發性腸穿孔早產兒的護理經驗

The Nursing Experience of Caring for a Premature Infant with Spontaneous Intestinal Perforation

摘要


本文旨在呈現一位29週早產兒罹者自發性腸穿孔的護理經驗。筆者自2005年11月6日至2006年1月6日的護理過程中,以系統性的觀察與會談方式,評估個案的生理狀態、行爲,並進行家庭評估,所確立個案的健康問題包括:低效性呼吸型態、皮膚完整性受損,案父母方面則有潛在危險性父母-嬰兒依附關係改變、知識缺失/早產兒照護。筆者於照護期間,藉由個別化護理措施,維持通氣功能的穩定與最佳氧合狀態,改善皮膚受損程度。針對案父母在照護早產兒的知識缺失及依附關係改變,運用家庭護理理念,與家庭成員共同制定照護計畫,增進案父母照顧個案的知識與技能,並建立親子依附關係。期望藉此照護經驗提供臨床護理人員在照護早產兒之參考,進而提升專業且持續性的照護品質。

This article described the nursing experiences of caring for a 29 weeks premature infant with spontaneous intestinal perforation. Data was collected by physical assessments, observations, and interviews from November 6, 2005 to January 6, 2006. The health problems of the patient included ineffective breathing pattern and skin integrity impaired. The health problems of the parents were impaired parent and infant attachment and knowledge deficit/caring premature. During the nursing period, the author applied individualized nursing interventions to help the client stabilize ventilation and to improve skin integrity. The author also used the concept of family care to promote parent-infant attachment and increase parents' knowledge and skills of taking care of a premature infant. The nursing experiences can help nurses to care for similar cases and improve quality of nursing care for premature infant.
