一位獨居老人接受半人工肩關節置換術手術全期護理之照護經驗
Perioperative Nursing Experience of a Living alone Elder with Hemi-shoulder Joint Replacement Surgery
摘要
本文旨在描述一位獨居老人接受半人工肩關節置換術手術全期護理之照護經驗。護理期間為2012年4月15日至4月24日，筆者在個案手術前病房訪視、手術期間直接照護，以及手術後藉由每天至病房探視，以會談、觀察、身體評估、查閱病歷等方式收集資料，確立個案主要的護理問題包括：焦慮、潛在危險性傷害、急性疼痛與身體活動功能障礙。筆者運用手術全期照護技巧，藉由手術前的訪視與個案建立良好的護病關係、連結醫護團隊支援系統，並主動關懷個案、鼓勵表達內心感受，提供適切的資訊以澄清個案疑慮，使個案手術前焦慮程度降低；在手術期間，由醫護團隊人員全面維護個案安全、提供完整性的照顧，使個案在手術過程中不致發生危險性傷害；手術後與恢復室和病房醫護團隊人員共同討論訂定，運用適當的疼痛護理措施，教導個案身體活動的技巧與注意事項，以減輕個案的術後疼痛，並協助個案逐漸恢復身體活動功能。最後，在個案出院時，提供並轉介可運用的社會資源，以協助個案出院後的居家照護及後續復健，使個案早日恢復原有的生活品質。

This article describes the perioperative nursing care experience of a live-alone elder undergoing hemi-shoulder joint replacement surgery. The nursing care period was from April 15 to April 24, 2012. Methods used to collect data were conversation, observation, physical evaluation and review of the patient's medical records. The nursing problems, including anxiety, potential risk of injury, acute pain and disability of physical activities were identified. The author helped reduce the patient's anxiety before the surgery by building a good relationship with the patient pre-operatively, connecting the supporting systems of health-care providers, taking care of the patient proactively, encouraging the patient to express his feelings, and providing proper information to clarify any doubt. During the operation, the author worked with the health care providers to maintain the security of the patient and to provide complete nursing care. The author also discussed the patient's pain relief and recovery of the patient's physical function after the surgery with health care providers in the ward. Finally, when the patient was released from the hospital, the author and the healthcare providers provided him social resources and helped him receive home care and rehabilitation, which let him restore his quality of life in a short time.
