摘要
合併性痣是一種獨特且具有混合性細胞的黑色素病灶。常見混合的細胞類型包括：普通痣細胞、黑色素紡錘狀細胞、聯藍色痣的樹枝狀細胞、及佔較少數的Spitz氏表皮狀及紡錘狀細胞。由於合併性痣從臨床上看起來很像普通痣，如果無法辨識出獨特的細胞型態表現，便無法與一般色素痣作區分而誤診。在此我們報告兩例分屬於不同亞型的合併性痣。在顯微鏡檢查下，第一個病例呈現的組織學型態，包含小型的先天性色素痣細胞夾雜肥厚的黑色素紡錘狀細胞；而另一個病例則是合併有普通複合性痣的細胞及藍色痣的樹枝狀細胞。我們所提出的病例，在經過一年的臨床追蹤後，一如過去絕大多數所報告過的病例。

Combined nevus is a unique pigmented lesion of mixed cytological patters. Among the usual attributes are ordinary (nevocellular) nevus cells, pigmented spindle cells, dendritic cells of blue nevus and occasionally epitheloid and spindle cells of Spitz nevus. It tends to be misdiagnosed for its common nevus appearance and its being indistinguishable from other melanocytic lesions without recognition of distinctive phenotypic expressions. We herein report two cases of variants of combined nevi. Microscopically, the first case shows a pattern of small congenital melanocytic nevus cells intermixed with plump, pigmented spindle cells, and the other case has a combination of compound nevocytic nevus cells with characteristic pigmented dendritic cells of blue nevi. Our cases like the overwhelming majorities of the previous reported cases, behave as a benign fashion without local recurrence or metastases during one year of clinical follow-up.
