Radiation Recall Dermatitis Induced By Actinomycin-D -A Case Report-

Actinomycin-D引起的回憶性放射性皮膚炎

摘要


回憶性放射性皮膚炎是由於投予某種藥物後,引起一種偶發的發反應,只侷限於先 前接受過放射線的範圍。吾人報告一個由actinomycin-D引起的回憶性放射性皮膚炎的病 例。一位三歲的男孩是一個威姆氏腫瘤併有肝臟、肺臟及淋巴結轉移的患者。在約一年前 施行右萬切除及化學治療。其後在數個月之間對右肺及肝臟作放射線治療,過程並無任何 合併症發生。在放射線治療結束約六週後給予含actinomycin-D之第二次化療,二天後在先 前作放射治療處發生界限分明的紅斑及脫屑。皮膚切片顯示界面性皮膚炎。在排除其它可 能的原因後,診斷為回憶性放射性皮膚炎。在口服及外用類固醇治療下,十天後紅斑逐漸 消退。(中華皮誌21:259-263, 2003)

The radiation recall dermatitis is an occasional inflammatory reaction limited to a previously irra- diated field when the patient is treated later with certain drugs. We reported a typical case of radiation recall dermatitis induced by actinomycin-D.A 3-year-old boy has Wilms’ tumor with hepatic, pul- monary and lymphatic meastases. He underwent right nephrecotomy and a course of chemotherapy one year ago. He received adiuvant radiotherapy for pulmonary and hepatic metastases in the follow- ing months. Six weeks after completion of radiotherapy, he received a second course of chemotherapy conaining actinomycin-D. Two days later, a well-demarcated erythematous patch developed right on the previous irradiation site. A skin biopsy revealed interface dermatitis. Other causes were excluded. After treatment with oral and topical steroids, the dermatitis subsided gradually 10 days later. (Dermatol sinica 21:259-263, 2003)
