第六型骨成形蛋白質於牙周組織再生之效果

Effects of Bone Morphogenetic Protein-6 in Periodontal Regeneration

摘要


自1960年代以來已由許多實驗證實骨成形蛋白質(Bone morphogenetic proteins, BMPs)能誘導新軟骨及骨骼的生成,其中在各類不同型之骨成形蛋白質(多為BMP-2、BMP-7)的傷口癒合實驗中大多有很好的效果;然而對於骨成形蛋白質第六型(BMP-6)在牙周組織再生方面的影響卻甚少被提出。本實驗的目的即欲探討利用合成BMP-6 polypeptides在大白鼠下顎區的開窗手術傷口中觀察其牙周組織再生的效果。實驗中選取約九個月大之實驗大白鼠共20隻,分為Control(0μg)、BMP-6 1μg、3μg and 10μg等四組(每組5隻),手術由下顎頰側經翻瓣後在臼齒牙根位置磨一缺損傷口並去除骨與牙骨質而深入至牙根之牙本質層,接著以可吸收性海綿狀膠原(absorbable collagen spongy)為載具放入四組材料後縫合,經28天犧牲動物並取標本做組織切片觀察。結果顯示在所有BMP組在缺陷區皆有明顯的新骨生長,尤其以3μg組為最多;同時BMP組具功能性牙周韌帶之新牙骨質也有顯著增加,同樣也以3μg組效果最好,且沒有任何牙根表面之髓連(ankylosis)現象被觀察到;對照組則幾乎沒有或只有些許之新牙周組織生成。因此我們建議BMP-6可藉促進膜內骨(intramembranous bone)之新骨與牙骨質的生成而為牙周病的治療上一種可適用的生長因子。

Since 1960, the roles of bone morphogenetic proteins in new bone formationrnve1een broadly reviewed. Several bone morphogenetic proteins (bone morphogenetic protein-2 or -7; BMP-2 or -7) have been shown a great potential in promoting wound healing. However, there is little information bout the specific spatial or effects on the periodontal regeneration of the BMP-6. The aim of this study was to investigate the effects of synthetic rat BMP-6 polypeptide on periodontal regeneration in a rat model. A total of 20 male SD rats, 9 weeks old, were ramdomly divided into four groups (5/group). In all animals, the buccal aspects of mandibular molar roots were denuded of the periodontal ligament overlying cementum and superficial dentin through a bony window created. Then, 0 (control), 1, 3 and 10 μg rrBMP-6 was delivered by using the absorbable collagen sponge delivery system. Animals were killed 28 days postoperatively and tissues processed for histological examination. Results showed that new bone formation was greater in the three BMP groups, especially in 3μg group, than that in control group. New cementum formation with functionally oriented collagen fibers had occurred in all BMP groups and was also stimulated significantly in 3 J1 g group. There was no any evidence of ankylosis was observed. On limitations of study, we suggest that BMP-6 might be a suitable growth factor for periodontal regeneration by enhancing the intra-membranous bone and cementum formation.
