某醫學中心教學門診病患就醫經驗初探

A Preliminary Study on Patients' Perception of a Teaching Clinic in a Medical Center

摘要


本研究目的為透過問卷調查方式,瞭解患者在教學門診看診之就醫經驗及實際參與教學門診後之就醫感受,資料收集時間為91年12月4日起至91年12月20日,共得89份有效問卷,回收率為87.89%。結果發現,病患來源主要是「以醫師約診」為主佔69.3%,其次為「複診,由掛號人員掛入」佔14.8%;是否知道為教學門診方面,有51.7%患者表示並不知情;有71.4%的患者為首次看教學門診,而第二次於教學門診看診的佔20.2%;候診時間部分有73%的患者表示適中,平均等候時間為25分鐘,而患者認為最合適之平均等候時間為13.5分鐘,顯示樣本醫院在候診時間部分仍有改善空間;看診時間部分有近九成(89.9%)的患者認為適中,僅9%的患者認為太長,平均看診時間為24.1分鐘,而患者所認為最合適的平均看診時間為19.5分鐘且有95%的患者表示有足夠的時間與醫師討論病情,表示樣本醫院在看診時間部分應該是受到患者所肯定的。能否接受學生在場部分,有88.8%的患者表示能接受學生在場,且教學門診中平均參與學生人數為2.54人,與患者可接受之平均學生人數2.16人相近,表示患者在學生參與部分是可接受的,但在「有學生在場是否會感到不自在」部分有75.3%的患者表示不自在;而開放性意見中部分患者反應意見主要包含四大類:未事先告知為教學診、有實習學生在場感到不自在、候診時間太久及看診時間太長等情形。

Patient satisfaction is a very important subject in clinical teaching. We performed a patient satisfaction survey by questionnaire to evaluate the outcome of our teaching clinics. The purpose of this study was to understand the level of patient satisfaction with the clinic and their perception of the teaching clinic. We collected 89 effective questionnaires from Dec.4 to Dec.20, 2002. The overall response rate was 87.89%. Most patients were selected by a physician (69.3%) and most of the others were invited by registration desk workers (14.8%). 51.7% patients were not informed that the clinic was a teaching clinic. For 71.4% of the patients, this was the first time they had attended a teaching clinic and for 20.2% patients, this was their second time. A majority of the patients (73%) considered the waiting time (24.94 minutes on average) was appropriate, while 13.4 minutes was considered to be better by the patients. The significant majority of the patients (90%) considered that the time the doctor spent on their case was appropriate (24.13 minutes on average), while 19.5 minutes was considered to be better. Most of the patients (88.8%) were able to accept the presence of the students at the clinic. The average number of students attending the clinic was 2.54, close to the number that the patient was able to accept (2.16). However, 75.3% patients were uneasy with the students at the clinic. In response to an open-ended question, the opinions fell into four categories: (a) there was no information in advance about the students attending the teaching clinic. (b) There were uneasy feelings about the presence of the students at the clinic. (c) The waiting time was too long. (d) The clinic session took too much time. Our study was aimed at not only understanding the level of satisfaction of the patients who attended the teaching clinic, but also at discovering the perception of these patients. Our study’s results should be able to serve as a reference for other teaching hospitals. We hope to establish a good teaching model that benefits the instructors, students, and patients, and encourages patients to join the teaching clinic positively. (Full text in Chinese)
