標準化病人訓練師資培育之週末短期工作坊

A Two Day Weekend Training Workshop for the Educators of Standardized Patients

摘要


應用標準化病人模擬臨床情境,以客觀地評估學習者的臨床能力,在國外已有40年以上的經驗,而於1998年首見於我國醫學教育期刊;隨後,許多醫學院及教學醫院亦紛紛將標準化病人運用於醫學教育領域。國內外研究雖已普遍肯定標準化病人在臨床醫學教育及臨床能力評估之有效性,然而標準化病人招募不易、訓練師資不足以及運用經費很高一直是我國標準化病人無法有效運用及推廣的主要原因。花蓮慈濟綜合醫院標準化病人中心引進美國標準化病人發展先驅-南伊利諾大學醫學院之訓練師資培育課程,並參酌國內外相關學術研究,建構了一套標準化病人訓練師資培育之週末短期課程。該培育課程包括角色扮演、教案寫作及訓練標準病人;透過理論的傳授、實務的演練以及經驗的交流,我們已舉辦3次工作坊,並訓練了22位來自不同學術機構、不同專業的訓練師資。這些成果顯示本課程如持續推廣,可以培育更多科別及領域的訓導師資,有效增進國內標準化病人之訓練師資。

Standardized patients (SPs) have been employed to simulate clinical encounters in order to assess objectively the clinical competence of a trainee for more than four decades. In 1998, the concept of SPs was first introduced into Taiwan. Since then, medical schools and teaching hospitals have implemented SPs as part of medical education. The usefulness of SPs in the teaching and evaluation of clinical skills has been well established through extensive research, but the application of SPs is hampered by difficulties in recruiting SPs, a lack of adequately educators of SPs and the expense of using SPs. In this article, we describe the introduction of a two days-weekend workshop for the training of educators of SPs. This training event was derived from the five day workshop at the School of Medicine, Southern Illinois University, which provided comprehensive concepts to help deal with potential difficulties. The workshop includes immersion, case development and how to train SPs. Using this workshop, the trainees were able to learn and carry out the main concepts involved in the educational process. Although most of the trainees were novice educators without any experience in the training of SPs, in three workshops we successfully completed the training of twenty-two standardized patient educators from different disciplines, who then returned to a variety of different medical institutions. Based on this initial success, we believe that this curriculum will be able to promote and improve the use of standardized patient programs in medical education in Taiwan.
