韓國國家醫事人員執照考試委員會:客觀結構式臨床考試之硬體設施與考試程序

Korean National Health Personnel Licensing Examination Board: Facilities and the Examination Process for the Objective Structured Clinical Examination

摘要


為因應考選部計劃以「通過高階客觀結構式臨床測驗(簡稱OSCE)」列為第二階段醫師國家考試的應考資格,台灣醫學教育學會特支助本文作者等全國11家醫學院的OSCE負責教師,於2009年11月9、10日前往位於韓國首爾的國家醫事人員執照考試委員會(簡稱NHPLEB)進行參訪。NHPLEB樓高七層,每層佔地大約百坪,其內有兩個考場,分別設於三樓及地下一樓。每一考場包括接待區、控制室、觀察區、CPX(clinical performance examination的簡稱,測驗考生與標準化病人[standardized patient,簡稱SP]互動,如問診、溝通及身體檢查的能力)考室六間、OSCE(測驗考生procedura1 skills的能力)考室六間及中央走道考區等。除了在各考室裝有影音系統之外,其餘設備和空間均頗為精間。由NHPLEB規劃的醫師執照考試舉辦臨床技能評估分為CPX、OSCE及inter-station(簡稱IS)三類,各佔六題。每天考生分三個梯次應考(考試開始時間分別為09:00、12:35及15:35),每梯次考試時間包括作答和等候共計157分鐘。每梯次每一考場可容納12人,兩個考場總計每天最多可容納72名考生應考。由於2009年度來自41所不同醫學院的考生人數有3539人,經規劃後訂於自2009年9月底至同年12月初期間的51個考試日考完。從NHPLEB提供的經驗,我們了解到舉辦high-stakes OSCE主要的成功因素是在流程的規劃與執行,而硬體設施只須達到基本需求即可。相信只要我們繼續努力和用心,OSCE必能順利成為台灣醫師國家考試的一環。

As the Ministry of Examination have planned to list ”a pass in a high-quality objective structured clinical examination (OSCE)” as an essential requirement for attending the second part of the National Medical Licensing Examination, the Taiwan Association of Medical Education decided to sponsor eleven teachers, including the authors of this article, to visit the Korean National Health Personnel Licensing Examination Board (NHPLEB) in November, 2009; this group included individuals from Taiwan's medical schools who will be responsible for the OSCE. The NHPLEB is located in a seven-story building and each floor has an area of approximately 300 square meters. Within the building there are two examination areas and these are located on the third floor and in the basement. Each examination area includes a reception area a control room, an observation area, six clinical performance examination (CPX) rooms, six OSCE rooms and a central aisle area. All examination rooms are equipped with audio-visual systems. The remaining equipment and space are quite streamlined. NHPLEB's clinical skills assessment is divided into three categories namely CPX to evaluate the candidates' competencies during interactions with standardized patients, OSCE to test the candidates' procedural skills and an inter-station test; each consists of six questions. There are three batches of tests each day with test start times at 09:00, 12:35 and 15:35. Each batch, including examination and waiting time, takes a total of 157 minutes. In each examination area, each batch can accommodate l2 candidates. Thus, in total, the NHPLEB can accommodate a maximum of 72 candidates a day. In 2009, a total of 3539 candidates from 41 medical schools attended the examination between September and early December, a total of 51 examination days. Based on the experience of the NHPLEB in this area, we now understand that planning and implementation are the key elements needed to create a successful high quality OSCE. In contrast to these factors, the demand for hardware facilities is not so critical. We shall continue our efforts because the development of an OSCE in Taiwan as part of our National Medical Licensing Examination is just around the corner.
