摘要

目的：本研究主要目的在探討女性憂鬱症病人的人格功能及敘事特徵。方法：以東部地區一所綜合醫院之身心科門診病人中，診斷為憂鬱相關疾患的女性病人，共計蒐集53名病人的資料納入統計分析。本研究採用「主題統覺測驗」(Thematic Apperception Test)以為蒐集病人在測驗上投射故事之工具。結果：病人的「現實考驗」功能較高，而「適應性退化」功能較弱；病人在TAT上所敘事的「故事情節」多以「平穩」（從「不好」到「不好」）為主，其「主體性」出現的次數相當低，而「共有性」則以關心／幫助一項出現次數最多；病人的人格功能與其敘事特徵具有部分的顯著相關。結論：女性憂鬱症病人的人格功能在本研究中普遍不具能力感，敘事中的「主體性」相當缺乏，故而在心理治療中應當多予「增能」；對於社會文化因素如何影響女性憂鬱病人，又與男性憂鬱病人之差異為何，這些都是未來尚待釐清的議題。

Objective: The main purpose of this study was to investigate personality functions and narrative characteristics among females with depression. Method: Fifty-three depressed female outpatients were included in this study. The projective ”Thematic Apperception Test” was used and dynamic and narrative analyses were applied for interpretation. Result: Among patients' personality functions, the highest mean score was for ”reality testing”, while ”adaptive regression in the service of the ego” was the lowest. On average, ”stable” narrative plots predominated while the characteristic of ”agency” was less prominent in their stories. Furthermore, narrative characteristics were significantly correlated with certain personality functions. Conclusion: This study showed that depressed female outpatients generally lacked a sense of their own competence; they therefore need to be ”empowered” in the course of therapy. This study suggests that the impact of social and cultural factors on women's depressive disorders and that comparison with depressive males merits further investigation.
