摘要

檢視我國整體社會發展趨勢，未來女性勞動參與率將逐漸臻至歐美國家高就業率水準，同時家庭結構核心化的結果，亦使擔任家庭內照護工作之主要人力萎縮。就倫理價值而言，居家及社區照護既能滿足個人及家庭沈重的照護需要，維繫及強化家庭的情感支持功能，亦將彌補機構照護較不易達成人性關懷的限制。再則，未來老人之教育與經濟狀況將較上一世代之老人為佳，勢必要求更符合人性品質之照護環境，目前之大型機構式照護模式實難以滿足其需要。因此，居家及社區照護將是未來長期照護政策的雙主流。本文係以未來之照護環境為考量，認為「就地老化」目標應為照護之規劃方向，而長期照護財務結構與給付水凖之設計需配合未來照護模式主流趨勢。

As the employment rate of women in Taiwan comes up, the main resource of long-term care is running out. At the same time, the increasing nuclear families also decrease the capacity to care the elders. As far as the ethic of care is concerned, aging in place undoubtedly fits the value to promote. It can not only connect and enforce the relationship of the family members, but also meet the humanitarian needs. Besides, the level of education and financial conditions among the elderly have been highly improved. Thus the model of grand scale of institutional care can't satisfy the needs of the elderly today and tomorrow. Home care and community care will be the mainstream of long-term care in the future. As a result, the design of long-term care financing framework and the level of benefit should take account of and match the main trend of care models.
