一位經產婦面臨安胎失敗之護理經驗

A Nursing Experience of Caring for a Multipara with Failure to Tocoysis

摘要

一位懷孕28週的經產婦，此胎因跌倒合併規則早期子宮收縮而住院安胎。個案住院期間，除承受著安胎藥物副作用造成的身體不適，同時擔心胎兒的健康情形及安胎失敗後的早產風險，短時間內身心承受龐大壓力。筆者於2010年8月1日至8月7日，在照顧個案期間，以生理、心理、社會、靈性四個層面作評估，確立護理問題包括焦慮／與期望安胎成功及擔心失敗有關、舒適型態改變／與子宮收縮不適有關、照顧者角色緊張／對早產兒照顧感覺壓力與不知所措，筆者藉由觀察、會談收集資料，協助個案瞭解安胎的重要事項、減少生理上的不適，並引導個案接受早產的結果及提供早產兒照顧相關資訊。

A multipara with pregnancy of 28 weeks was admitted to the hospital for tocolysis because of regular contractions after a fall. The patient suffered overwhelmed physical and psychological stress from the adverse effects of tocolytics and worried about the baby's health and the risk of preterm labor. The nursing period was from August 1 to 7, 2010. The author assessed the patient's needs in physiological, psychological, social, and spiritual aspects. The identified nursing problems included tocolysis-related anxiety, contractions of uterus-associated discomfort, and the caregiver's stress of caring for the premature baby. Through observation, interview, and data collection, the author assisted the patient in understanding the importance of tocolysis, minimizing physiological discomfort, and providing guidance in accepting the preterm labor outcome by offering information on how to care for premature babies.
