Painful bulging mass over the female exogenitalia is frequently seen in gynecological clinics. One of the differential diagnoses is urethral diverticula. Needle aspiration of other cystic lesions over exogenitalia to relieve tension and pain is a common and effective treatment. However, mistaken repeated needle aspiration of urethral diverticulum may induce infections and sometimes lead to the formation of a urethral fistula. Herein, we reported a 49-year-old female patient who suffered from urine leakage due to such a urethral fistula. After the treatments of urethral diverticulectomy and fistulectomy, her urine leakage was completely cured.
