高齡者身心機能活化運動成效評估

The Activating Exercise Program for the Psychomotor Function of the Elderly: An Evaluation of the Effect

隨著老年人口比例逐年提高,長者對安養機構的需求日增。護理之家除了提供必要的醫療服務外,住民的生活品質更是護理之家的重要課題。為了讓長者晚年生活更有樂趣,規劃長者所需之休閒活動更是刻不容緩。身心機能活化運動是一套針對高齡者設計的活動,在日本已實施多年,且頗受好評。本研究期望探討身心機能活化運動對於提昇護理之家的老人們身心功能的效果。 研究對象為護理之家住民,年齡65歲以上(平均81.40±.43歲),日常生活功能達巴氏量表得分20分以上者,共20人。研究目的是在瞭解護理之家住民參與身心機能活化運動後在認知功能、日常活動功能及整體生活功能之變化,以及住民參與機能活化運動的表現與各項功能的相關性。使用單一樣本設計進行研究,以介入前一個月的評量為基準,以後每個月評量一次。介入為每週兩次的身心機能活化運動,介入二個月後暫停介入一個月,之後再繼續介入一個月,總計評量五次。研究結果顯示:(1)住民參與機能活化運動後,僅在老年人生活功能量表中的夜晚睡眠或活動情形上及認知功能上有明顯不同,呈逐漸進步的趨勢,(2)住民參與機能活化運動的項目內容及多寡因能力不同而有所不同,但各項評量結果並無明顯不同,(3)住民對機能活化運動之參與程度及參與時之情緒表現與住民的認知功能及其活動時的動作表現有關,住民的認知功能或動作表現較佳者,其活動參與度與情緒反應也較好。 綜合而言,身心機能活化運動對改善護理之家的老年人之身心功能狀況效果有限。除活化運動本身,活動進行的方式亦可能是影響效果的因素。此外,後續研究宜增加研究對象或採用對照組之實驗設計,將更提高研究結果的可信度。

The demand of long-term care facilities has increased as the population rate of elderly people increase yearly. In addition to providing necessary medical services, improving the quality of life for the residents would be the focus for skilled nursing facilities. The activating exercise program for psychomotor function is an activity program designed for the elderlys. In Japan, it has been used for many years and has earned good reputation. The purpose of this study was to investigate the effect of this activity program in promoting the physical as well as psychological function of the elderly residents in nursing home facilities. The participants were 20 residents of a nursing home aged 65 or above (mean=81.40 祥.43 years). Their functional level of activities of daily living (ADL) rated by Barthel Index were all above 20. A single subject design was adopted. The activating exercises was first provided twice a week to all the participants for three months in total with one month recess after the first two months. The changes of cognitive function, ADL function and the whole living function of the participants were compared before and after engaging in the activating exercises to investigate the effect of the activating exercises for psychomotor function. The correlations between the subject's performance in activating exercises and their cognitive, ADL and whole living functions were also calculated. All measures were collected one month before intervention to serve as baseline. All the measures were re-collected once a month. The results showed that 1) after participating the activating exercises, the residents gradually improved in cognitive function and behavior at night, while the other functions remained the same, 2) although the activating exercises each subject participated varied according to their abilities, the variability of activities made no differences on the results, 3) the involvement levels and emotional responses to the activating exercises of the subjects were positively related to their cognitive function and the quality of their motor performance. In summary, the effects of activating exercises for improving the psychomotor function of the elderly residents in nursing home facilities were limited. In addition to the activities designed, the way of carrying out these activities might also influence its effect. Further research should recruit a larger sample size or adopt a comparison group to increase the reliability of the results.
