許多已開發國家的研究已證實喘息服務具有減低照顧者壓力、增進照顧者健康等功能,並且對影響喘息服務利用的因素進行探討。本研究目的在瞭解北台灣家庭照顧者使用喘息服務的現況與相關影響因素。本研究採橫斷調查研究法,以北部四家居家護理服務對象之主要照顧者為調查樣本,以面對面訪談、填寫問卷的方式收集資料。有意願參與研究者有140人,完訪人數為100人(完訪率71.4%)。研究結果包括:(1)65%的照顧者完全不知道有喘息服務,僅有9%自認為很清楚。有使用過喘息服務者占11%,未來有使用意願者占43%,有46%的主要照顧者表示各類喘息服務均不符合他們的需要,居家照顧符合最多主要照顧者的需要。(2)是否使用喘息服務與照顧者年齡、家庭型態、居住地區、對喘息服務的認識及喘息服務類型是否符合其需求有關。(3)不使用喘息服務主要原因有品質問題、義務與情感因素、運輸困難、經濟考量、特殊照顧需求、病人及家人反對等。根據研究結果建議,未來的政策應增加照顧者對喘息服務的認識,並在提供喘息服務時,先優考量照顧者的實際需要,及解除家庭照顧者使用喘息服務的疑慮。
Many research findings have suggested that respite care might alleviate the stress of caregivers and enhance their health conditions in developed countries. The purposes of this research were to investigate the utilization of and attitudes about of respite care among family caregivers, and to examine factors related to their willingness to utilize such care. A Cross-sectional study was conducted with a convenient sample of 100 family caregivers from 4 home care agencies in North Taipei. The researcher(s) used a structural questionnaire in a face-to-face interview with caregivers in home settings. Results of this research included: 1. Sixty-five percent of caregivers didn't know about respite care service; only 9% caregivers fully realized it. The utilization rate of respite care was 11%; 43% of caregivers expressed willingness to use respite services in the future, and 46% of caregivers didn't think that their needs could be met through the different kinds of respite services available. Home care respite was considered the fittest type by caregivers. 2. There were significant differences between caregivers who used and didn't use respite care services in terms of the caregivers' age, family type, living location, recognition of respite, and whether respite care services fit the caregivers' needs. 3. Issues related to quality of services, obligation and affection, transportation, financial concerns, special nursing needs, and conflicts with other family members and the patient were the main concerns among non-users. Results suggest that promoting recognition of services among caregivers should be considered in future respite care; caregivers' needs are first priority and their concerns about respite care services should be resolved when providing these services.
