摘要

對臥床個案而言，良好的照顧技能是預防合併症的重要工作。本單位居家護理師常表示，外籍看護工（簡稱外傭）對臥床個案之管路及皮膚照顧技能執行不正確，而因語言溝通障礙需花費許多指導時間，但不知外傭是否已完全了解指導內容，故成立專案小組以了解外傭照顧技能現況及問題，於2008年5至7月查核本單位40位外傭（佔機構總外傭比例40%，N＝100）對胃管、尿管和皮膚照顧之技能正確率僅74.9%，經現況分析得知其原因為：單位缺乏外傭照顧技能指導之工作標準、外傭照顧技能評值工具不完整、護理師缺乏外傭照顧技能指導之認知、語言溝通障礙及指導輔助工具不合適等。故以制定外傭照顧技能指導之工作標準、增訂外傭照顧技能評值表及評核機制、進行護理師之外傭照顧指導在職教育、建立外語指導手冊，並回覆示教指導，經過5個月改善措施之執行，結果顯示：外傭照顧技能總正確率提升至94.0%，達專案目標。本專案解決問題之方法不僅提升外傭照顧技能，進而使居家個案獲得良好的照顧品質，希望藉此專案的執行經驗與居家護理同仁分享。

Good quality of care is the most important part in preventing complications for bed ridden patients. Home-care nurses in our hospital often complain that most of the foreign caretakers cannot provide correct practice on caring naso-gastric/ foley tube and injured skin for the bed ridden patients. Because of the language barrier, home-care nurses need to use more time for education foreign caretakers but it is difficult to check how they understand.
In order to realize the current condition and problems for foreign caretakers, we set up a special research team. From 2008 May to July, we evaluated 40 foreign caretakers in our institute about their practice on caring naso-gastric/ foley tube and injured skin for the bed ridden patients and we found the accurate rate was only 74.9%. The possible causes are lacking standards for education and evaluation form for the foreign caretakers. All the home-care nurses didn't know how to educate them and they got great language barriers.
So, we set up the standard for education and evaluation form for the foreign caretakers, and we started the training courses for training foreign caretakers with an education manual in foreign languages. After five months later, the accurate rate was improved to 94%. We did this program for improving practice for foreign caretakers and prevent complications for our patients and we hope the experience can be shared to all the other institutes.
