台灣長期照護機構品質確保機制發展之趨勢

摘要

長期照護品質攸關失能者的生活品質，過往長期照護機構因誘因不足及照護人員照護品質相關知能不足，失能者往往難以得到適切的服務，民眾對於長期照護機構服務品質也日漸重視。我國長期照護品質確保系統發展現況及未來發展模式，過去較少人關注；本文從品質管理之法律定位、入住（收案）評估系統、長期照護機構評鑑、長期照護品質指標及人員的資格與教育訓練五大面向，探討國內品質確保系統之發展及國外先進國家作法，供相關單位及機構經營者參考；並對未來品質確保系統之發展方向提出五點建議：整合長期照護機構管理法源納入品質管理法規、建立全國一致性入住（收案）標準化評估資訊系統、整合各類長期照護機構評鑑制度、逐步增加其他重要長期照護品質指標，以及建立全國一致的人員資格與訓練。

The life quality of disables is related to the quality of long-term care. In the past, the disables could not receive adequate care, because of no incentive of long-term care facilities, and less care ability of care givers. The request of care quality about long-term care facilities has been more concerned gradually. However, long-term care quality assurance system development and future development model are less concerned for the past in Taiwan. This article investigates the development of quality assurance in Taiwan and the experience of developed counties, to share to some relative public agencies and facilities managers. Those investigations are including five topics such as legislation of quality assurance, enrolled client disability assessment system, long-term care facilities accreditation, care quality indicators, and personnel qualification and training program. Five suggestions for the quality assurance in the future are integrating different legislations of long-term care facilities with quality assurance regulations; establishment of national consistency of assessment tools for the enrolled clients; unified accreditation for different type of long-term care facilities; gradually introducing other important long-term care quality indicators; and creating a national personnel qualifications system and training programs.
