運用歐倫護理理論照護一位創傷性腦損傷患者復健期之護理經驗

摘要

本文係敘述照顧一位因創傷性腦損傷的患者在復健期之護理經驗。護理期間自2008年7月23日至8月20日，筆者以會談、身體評估、觀察方式收集資料，經由歐倫(Orem)護理理論之評估，確立個案有身體活動功能障礙、自我照顧能力缺失（如廁、個人衛生及移位）、潛在危險性損傷之護理問題；主要照顧者在缺乏社會支持情況下，產生照顧者角色緊張。筆者運用歐倫理論之完全代償、部分代償及支持教育護理系統等概念提供個案各項護理措施，加強訓練個案日常生活自我照顧技巧，護理過程中也藉由與個案及家屬的良好互動及會談，提供正向的支持、鼓勵並及轉介合宜的社會資源，協助個案及家屬緩解照顧者的角色緊張。因此，希望藉由此篇個案報告提出分享，期盼能對創傷性腦損傷復健期患者之照護有所裨益。

The aim of this article was to describe a nursing experience for application of Orem's self-care Theory to care of a patient with traumatic brain injury in the rehabilitation stage. During the caring period between July 23, 2008 and August 20, 2008, the author used face-to-face interview, physical assessment and observational methodology to collect the data .The results demonstrate that the patient had physical impairment, self-care deficit and high risk for injury problems. The primary caregiver was unable to cope well with stress because of lack the social support. The author used Orem's Nursing System including the wholly compensatory system, the partially compensatory system and supportive education to provide nursing care intervention. Based on these problems, we strengthened the patient's self-care skills, during the nursing process, we provided the caregivers with information about the illness care knowledge, positive support, encouragement and referred of social resources to help the both care and his family to relieve caregiver role’s strain. Therefore, hopefully this case report can provide information for clinical nursing care in a patient with traumatic brain injury in the rehabilitation stage.
