降低護理之家住民跌倒發生率之改善方案

A Program for Reducing Falls in Nursing Home Residents

摘要


老人發生跌倒意外容易造成住民身體損傷,嚴重者甚至死亡,故預防跌倒意外的發生是護理之家重要的議題。透過病人安全通報統計,本單位於2008年住民跌倒發生率為0.08%,造成中重度傷害有2件,且有逐年增加趨勢。故成立專案小組,發現病人跌倒的原因有工作人員未依流程作業、對跌倒認知不足及住民身體活動功能障礙,藉由改善方案:修訂預防跌倒照護標準,改變跌倒告示牌樣式及標示、步態不穩及虛弱之住民,有定時主動協助在床上或床邊使用便器、在職教育宣導及作業流程的查核,住民跌倒發生率由0.08%降至方案執行後的0.05%,因跌倒造成的中重度傷害降至0件。持續的跌倒監測、檢討與改善,定能增進住民安全,提昇單位的照護品質。

Fall accidents can easily cause injuries or even death in the older adults. Preventing occurrence of fall accidents is an important management issue for nursing homes. According to the statistics of residents' safety surveillance in our nursing home, the incident rate of residents' fall accidents was 0.08% in 2008. Two of all events were moderate to severe injuries, and the incident rate increases annually. Therefore, we established a special committee to design a fall prevention program. It was observed that the causes of falls were: not following a standardized flowchart, insufficient knowledge of fall prevention, and the functional disability of physical activity. The incidence of falls decreased from 0.08% to 0.05% and moderate to severe injury declined to one event after the program was implemented. The program included revision of a standardized care of fall prevention, changing of the style of fall prevention billboard, and application of care manpower board, continuing education, and audit of operating flowchart. Monitoring all falls continuously, self-criticism and finding ways to improve can surely promote safety of residents and the quality of care of nursing homes.
