摘要

長期照護機構若要讓住民在機構善終，需瞭解社會政策、機構環境、住民特性，及兼顧安寧療護的精神意義，從全人、全隊、全家、全程各面向去評估住民的需求，並視機構內部與外部安寧療護資源協調整合，以提供適當的服務，方能達成在機構終老的目標。這正是長期照護機構安寧療護的社會心理照護所擴及範疇，本文希望透過相關議題的探討，提出長期照護機構在提供安寧療護社會心理照護服務時可行的方向，企盼每一個住民都有機會選擇他們善終的途徑與方式。

To implement hospice and palliative care, long-term care systems need to be adjusted to related social policies, institutional environments, and resident characteristics. It is important to assess the need of each resident based on values of good death to take care the whole person and their families with the continuing services provided from multiple-professional teams. It is also crucial to coordinate internal and external hospice and palliative care resources in long-term care institutions in order to provide appropriate psychosocial services and reach the goal of good death in home-like institutions where the residents live. The purpose of the article was to explore related issues through the proposed long-term care institutions in the provision of hospice services. We hope every resident has the opportunity to have a better understanding of palliative care in his/her dying stage, express his/her living will, and make a meaningful decision for himself/herself.
