傳統儒家文化薰陶下的「失智症照護」－以健順體系中山日照間照顧中心為例

摘要

近年來，關於失智症對於家庭照顧者影響的相關文獻討論眾多，主要引用「壓力與生活調適」模式作為分析架構，然，以實務工作照顧服務過程中察覺到，照顧服務提供的抉擇，同時也受到傳統儒家文化價值所影響。爰此，本文從文化架構去探究健順體系之中山日間照顧中心的服務使用者、服務提供團隊（包含護理師和社工師）在失智症照顧的經驗。主要發現如下：一、服務團隊可打破性別文化的束縛，再造性別的價值；二、不同的家庭照顧者的身分（譬如配偶、兒子、女兒、媳婦等），可從照顧提供上了解到其受到文化期待所影響；三、服務提供團隊可運用環境文化與習俗文化，拉近與失智症者距離。最後，依據失智症照顧經驗提出建議提供給服務提供者與照顧者參考，期待能夠減少失智照顧上的負荷。

In recent years, lots of researches and studies emphasized on the influence of dementia on family caregiver, and analyzed it based on ＂stress and coping strategy.＂ However, at the scene of care process, the care decision is influenced by Confucian value at the same time. According to this concept, this study probe to the experiences of the family caregivers and daycare team members (including the nurses and social workers of Zhongshan daycare center of Jiangsu system) of dementia by the interviews of them. The conclusions are listed as below: (1)The care team should dispel the gender stereotypes and reconstruct the value of gender. (2)The different identities of family care responsibilities (e.g., spouse , son, daughter, and daughter in law) would cause the different care decisions of them according to the traditional culture exceptions. (3)The daycare team members could utilize the feature of environmental and traditional culture to get close to the dementia elders. At the end, according to the results, this study would provide some implications for caregivers who provide the care for elders, and expect to reduce the burden of caregiving.
