預防及延緩失能服務於社區推動之實務與課題－以臺南YMCA為例

臺灣的高齡化正迅速進行中，預計在2025年即將達到超高齡社會。2017年長期照顧十年計畫2.0擘畫出臺灣未來十年高齡政策，除有多元連續服務與普及照顧服務體系，更重要的是跳脫從解決長期照護問題，延伸到前端初級預防功能，重推動預防保健、活力老化到減緩失能，與銜接後端如在宅醫療等服務，乃為迎合國際發展趨勢之作法。其預防延緩服務計畫強調成效目標的服務介入與實證或研究根據，以不同專業介入為基礎的方案模組，已不同於以往社區中多以休閒娛樂性質活動而難以得知其成效問題，此乃為社區活動品質提升上邁前一大步。本文介紹日本推動社區預防照護活動經驗，並說明臺南YMCA參考日本經驗於社區推動照護預防之執行過程，以及目前面臨在參與意願、社區據點量能不均及有限補助經費下運作的課題，提出未來之展望：落實於社區整體照顧服務模式中、在地社區照護預防人力的永續性培育、以社區民眾為主發展的照護預防工作，以作為日後推動之參考。

In Taiwan, aging population is increasing rapidly and will be the Super Aged Society in 2025. The Long-term Care 2.0 constructs the ten-year Aged policy in Taiwan in the future. Besides continuum services and universal care service system, it's very important to have prevention to delay disability upstream service and to connect downstream services, for example home-based medical care, not just only to solve the long-term care problems. To pay attention to preventive health care, active aging and delay disability is the international trend. However, there're a lot of programs just for fun in the community now, t's not easy to know the effectiveness. The programs to prevent or delay disability emphasize effectiveness-oriented services and researches by different professionals and programs. By the way, it improves the quality in the community. First, the article will interduce the prevent programs in the community in Japan, and by the experience to apply in Tainan YMCA. Now, the challenges are the aged participate in will, the shortage of the community in the quantity and ability, and limited fund. Finally, to implement the programs in the comprehensive community care system, to train the community-based prevent program providers continuously, and to develop community-based prevent programs will be helpful to promote in the future.
