本研究的目的在探討從Orem的自我照顧觀點來看乳癌高危險群婦女,其日常生活中,執行健康促進及預防性乳房自我檢查的行為及其間的關係。以基隆地區衛生所轉介到區域醫院之65位乳癌高危險群婦女為研究對象,研究設計採橫斷面問卷訪查方式進行資料收集。研究結果為:(1)乳癌高危險群婦女沒有規律執行乳房自我檢查的比率仍高(44.6%),原因以不知道如何做居多(69.0%),且先前不知道自己是屬於高危險群的比率亦高(42%)。執行方法的獲得是經由醫護人員結合大眾傳播媒體的比率較高(61%),單純由醫護人員提供者則佔33.3%;(2)在健康促進行為的六大項目中,以「人際間的支持」之項目得分最高,而以「健康責任」之項目得分最低,其中有七項行為頻率是介於“從未”執行到“有時”之間,(3)有執行乳房自我檢查者,其「運動」項目中之行為得分及整體健康促進行為總分,皆比未執行乳房自我檢查者為佳,且達統計之顯著差異,(4)篩檢後經進一步診斷過程發現有16(24.7%)人確實呈現有腫塊異常問題。這些結果可提供衛生所和區域教學醫院合作從事婦女健康促進有價值的參考資料。
This study investigated the relationship between the practicing of Breast-Self-Examination (BSE) behaviors and related health-promoting behaviors in women who have a high risk of breast cancer. The theoretical foundation of this study is Orem、s self-care theory. This cross-sectional survey included 65 women with a high risk of breast cancer, and who were referred from Keelung local public health agencies to a teaching hospital. Results showed that (1) the rates of practicing SSE in the high-risk breast cancer group were not high (44.6%) and (hat the primary reason why BSE was not practiced was that the women did not know how to do so (69.0%); nearly half (42%) of them did not know they were candidates for breast cancer Approximately one- third (33.3%) of the subjects were taught and counseled by medical personnel on how to employ BSE techniques. The BSE method was communicated to a high percentage of the women (61 %) when consultations with medical personnel were combined with multi-media approaches. (2) Among the six dimensions of the health-promoting behaviors, interpersonal supportive behavior scored most highly while health-responsibility behaviors scored lowest. Seven health-enhancing behaviors were practiced with a frequency between “never” and ”sometimes”. (3) Those who practiced BSE obtained significantly higher total scores and scores for exercise, than the group of women who did not practice BSE. (4) Finally, 16 (24.7%) women were found to suffer from a breast mass after this screening process. These results support the claim that activating a partnership between local health agencies and teaching hospitals was valuable.
