糖尿病個案之賦權內涵: 一個質性導向的研究

摘要


背景 賦權是抽象的概念,要確實執行病人賦權是不容易的。賦權需要賦權者與被賦權者彼此參與,且賦權的內涵會因不同文化背景而有所不同,故需要以本土化的角度,且同時納入賦權者與被賦權者的觀點以完整呈現賦權內涵。目的 本研究目的為從糖尿病衛教師與糖尿病個案的觀點,探討國內糖尿病個案的賦權內涵。方法 採質性研究法,在2009年4月至6月南部3家設置有糖尿病共同照護網之醫院,以半結構式問卷訪問對象收集資料,共收取12位糖尿病個案與7位糖尿病衛教師的訪談資料,並採用內容分析法加以分析歸納。結果 賦權內涵包含三個階段及其主題如下:(一)察覺階段,包括伙伴關係的建立及喚醒與糖尿病同生共存;(二)行動階段,包括充能及權力的轉移;(三)評價階段,包括共同評值及回饋個案。結論 從傳統的衛教方式轉移至賦權衛教方式是很不容易的,深入瞭解賦權內涵有助於健康專業人員執行賦權策略。本研究分析糖尿病衛教師與糖尿病個案對賦權內涵的觀點,期結果能作為日後糖尿病照護的參考。

Background: Empowerment is an abstract concept that is difficult to practice. Empowerment requires mutual participation between empowerer and empoweree. Empowerment may differ from the cont ext. Therefore, it is necessary to explore the context of empowerment in Taiwan. Purpose: The purpose of this study was to investigate the content of empowerment from the perspectives of people living with diabetes and educators.Methods: Data collection in this qualitative study was per formed by semi-structured interviews conducted from April to June, 2009. A convenience sample o f 12 people living with diabetes and seven diabetes educators were recruited from the endocrinology outpatient clinics o f three hospitals in Kaohsiung, Taiwan. The data were analyzed by content analysis.Results: Three phases of patient empowerment and two categories of each phase emerged from the interview data. The awareness phase included partnership building between empowerer and empoweree and their co-discovery of empowerment issues that arise in people living with diabetes. The action phase included enrichment of the patient and shift of power from empowerer to empoweree. The evaluation phase included reflection and feedback.Conclusions: Healthcare professionals often face difficulty from the traditional education shifting to empowerment education. Therefore, they are required to understand the cont ext of empowerment. The percept ions of people living with diabetes and educators elucidated in this study provide a valuable reference for diabetes care.
