一位感染呼吸道融合病毒導致肺炎早產兒之護理經驗

Nursing Care of a Preterm Infant with Respiratory Syncytial Virus (RSV) Pneumonia

摘要


本文探討一位感染呼吸道融合病毒導致肺炎早產兒之住院護理經驗,護理期間為2008年9月6日至10月7日,透過觀察與非結構性訪談進行資料收集與分析,運用Neuman健康照護系統評估模式為評估工具,探討個案及案母於住院期間所面臨的健康問題與需求,結果發現個案有氣體交換障礙與混亂性嬰兒行為護理問題,及母親焦慮情緒反應。個案住院期間配合醫療處置與加強胸腔物理治療,改善肺炎導致之氣體交換障礙問題,藉新生兒個別化發展性照護措施與疼痛控制有效穩定個案情緒與行為反應。案母方面主要護理問題為焦慮,導致生理方面及人際關係的問題,透過有效溝通瞭解其想法與感受,給予心理支持,共同討論排定個案居家照顧知識與技巧教導時間與方式,經實際操作各項照護技巧,母親顯著提升養育個案之信心。

This case report explores the physiological and psychosocial needs of a mother who had a preterm infant (gestation age: 29 weeks) with Respiratory Syncytial Virus (RSV) Pneumonia in a neonatal intensive care unit (NICU). The period of care ran from September 6 to October 7, 2008. The author collected data regarding the infant's nursing care problems and maternal needs by observation and non-structured interview. Neuman's system model was used to explore the infant's problems and mother's needs as well. The results showed that the infant had gas exchange impaired and behavior disorganized; the infant's mother had anxiety problems. The researcher provided medical management and chest care to improve gas exchange impaired for pneumonia during the hospitalization. In addition, in this research, using newborn individualized developmental care and pain control stabilized the infant's emotion and behavior reaction. The researcher also built a firm and true relationship with the infant's mother, providing support through full communication. From mutual discussion and demonstration, the mother learned the knowledge and skill of preterm infant's home care. Thus, the infant's condition and parenting confidence improved.
