摘要

本文運用關懷照護理論為基礎，護理一位過敏性肉芽腫症候群個案接受體外膜氧合治療導致周邊動脈阻塞疾病面臨截肢之經驗。護理期間為101年7月25日至101年8月11日，筆者藉由直接照護、觀察與會談方式收集資料，針對個案身、心、社會與靈性各層面評估，發現有氣體交換功能障礙、急性疼痛及身體心像紊亂等問題。照護過程中，監測生命與組織缺氧徵象改善氣體交換障礙情形；止痛劑使用並輔以音樂與轉移注意力減輕疼痛達成生理舒適與健康；提供家人與社會關懷照護截肢後身體心像紊亂衝擊，增強回歸社會的復原力。過敏性肉芽腫症候群為臨床罕見疾病，然國內缺少此類個案探討文獻，分享此照護經驗供日後照護類似個案參考。

This paper presented a churg-strauss syndrome patient complicated with respiratory distress with temporal use of ECMO life supporting system in ICU. The nursing period was from July 25 to August 11, 2012. The data of the study was collected by the direct nursing care, observation, and interview by authors. Watson’s caring theory was applied in the period of nursing care, and three health problems including poor gas exchange, acute pain and body image disturbance were established. This successful nursing experience provides information that might be of use when caring for patients undergoing ECMO treatment in a critical setting.
