降低一般外科病房中心導管相關血流感染率

中心導管相關血流感染是最常見且重要的醫療照護感染指標之一，本單位2017年1月1日至4月30日平均中心導管相關血流感染率為（千分之5.15），相較於其他同屬性病房（（千分之1.47-1.79））高出3～4倍，引發專案小組改善動機。經由現況調查發現中心靜脈導管置放過程與護理人員執行中心靜脈導管照護不完整。經由舉辦教育訓練、製作宣導海報、提供計時器、設置合適材質手套及建立自主稽核制度等介入措施，使得中心導管相關血流感染率降為（千分之2.64）、執行中心靜脈導管放置過程完整率由70%提升至97%、中心靜脈導管照護完整率由55%提升至93%，達成本專案目標值，落實標準醫護技術於工作中，可提升醫療照護品質與病人安全。

Central-line associated bloodstream infection is one of the most common and important indicators of healthcare-associated infection. The average central-line associated bloodstream infection rate in our unit from January 1^(st) to April 30^(th), 2017 was (5.15 per mille), which was three to four times higher than other similar wards ((1.47-1.79 per mille)). The investigation found that the doctor placing the central venous catheter process and the nurse caring central venous catheter were incomplete. After the holding of education and training, production of posters, provision of timers, setting of appropriate material gloves, and establishment of an independent audit system, the central-line associated bloodstream infection rate decreased to (2.64 per mille), and the complete rate of placing the central venous catheter increased from 70% to 97%. The complete rate of nursing care on central venous catheter increased from 55% to 93%, achieving the goals of this project. The implementation of standard medical care technology at work could improve care quality and patient safety.
