中風病人出院預後功能恢復的追蹤研究－以南投縣社區居民為基礎之初步調查報告

A Follow-up Study of the on Functional Recovery of Stroke Patients after Hospital Discharge: A Preliminary Report Based on Community Study in Nan-Tou County

摘要

研究背景與目的：本研究目的是追蹤調查腦中風患者接受住院治療出院預後日常生活功能恢復的情形，並且進一步評估居家物理治療的可行性。方法：採用以社區居民為基礎的橫斷式研究調查法，首先從南投縣社會局身心障礙人口電腦系統找出在民國89年期間經鑑定領有身心障礙手冊的腦中風患者為調查對象。結果：鑑定有效完整資料共543份，身心障礙等級判定為極重度、重度、中度及輕度者，分別有46位(8.5%)、155位(28.5%)、262位(48.3%)、及80位(14.7%)。完成電話訪談者共計有251位，日常生活功能獨立自主者有36位，由家人照護者有105位，請看護或外籍監護工在家照護者有33位，住進長期照護機構者有10位，4位因疾病住院中，63位已經死亡。完成居家訪視評估者，共計25位，有14位被建議需要給予居家物理治療。結論，相關結果可能有助於縣政府擬定醫療福利政策和繼續執行重建區「復健醫療專案」的參考。未來經由效益性評估，希望建立偏遠鄉鎮地區居家物理治療可行性模式。

Background and Purpose: The purpose of this study was to investigate the activities of daily living of functional recovery among stroke patients after hospital discharge, as well as to evaluate the feasibility of physical therapy at home. Methods: This cross-sectional survey study based on community in Nan-Tou County was conducted in a cohort of stroke patients, who were enrolled to a handicap list of computer system at the Bureau of Society during the year of 2000. Results: Of the 543 subjects surveyed, there were 46 patients (8.5%), 155 patients (28.5%), 262 patients (48.3%), and 80 patients (14.7%) demonstrated the degree of accreditation with very severely, severely, moderately, and mildly, respectively. Among 251 subjects based on phone interview, 36 patients lived independently at home, 138 still lived at their own home dependent upon their family members (105) or paid carers (33), 14 patients resided in a long-term care institution (10) or rehospitalization (4), and 63 died before phone interview. At the time of the follow-up visit, 14 of 25 patients were suggested to receive physical therapy at home. Conclusion: The results may assist in setting up relevant socio-welfare policies and providing continuous rehabilitation care plan for stroke victim at community in Nan-Tou. In the future, the effect on physical therapy at home will be verified in order to establish the feasible mode for stroke survivors at rural area.
