以整合治療介入跆拳道運動員慢性下背痛之成效:個案報告

The Effectiveness of Integrated Intervention for Chronic Low Back Pain in a Taekwondo Athlete: A Case Report

摘要


背景與目的:慢性下背痛指下背及腰部疼痛超過三個月以上且難以康復,本研究以整合治療介入跆拳道個案評估療效。方法:一名跆拳道女性菁英運動員,先前每週訓練27小時,背痛期間仍積極訓練,踢擊過程中伴隨軀幹扭轉,導致腰部痠痛致使許多動作受限制而無法久坐超過10~15分鐘,嘗試各種療法無明顯改善,嚴重影響日常生活,進而求助物理治療。治療前疼痛指數高達10分,反覆疼痛達半年以上。本研究於不同階段給予合適介入,並進行療效評估,治療前填寫視覺疼痛量表及SF-36健康量表問卷(Short Form-36 Health Survey, SF-36),給予熱敷、電療及徒手治療,第四週加入穩定性訓練,第八週進行動作模式及姿勢再教育,治療頻率為3~4次/週,每四週監測疼痛指數,共計12週。結果:疼痛指數由10分降至3分,坐立進步至30~45分鐘以上,SF-36顯示身體疼痛、活力狀態、社會功能、心理健康皆明顯改善。結論:本研究採整合治療方式介入跆拳道專項個案,善用實證醫學理念改善個案疼痛,供臨床治療參考。

Background and Purpose: The symptoms of chronic low back pain (CLBP) can last for more than three months and hard to recover. This case study reported the effectiveness of integrated intervention for CLBP in a female taekwondo athlete. Methods: A female elite taekwondo athlete presented to the physical therapist with a half-year's history of intermittent non-specific low-back pain of severe intensity (Visual Analogue Scale [VAS] 10) after folk healing and traditional medicine failed. She was enthusiastic about taekwondo training for competition, even with the presence of low back pain (LBP). The big range of torso rotation during kick movement in taekwondo was greatly limited by LBP, and the pain worsened to the stage that she could not sit still for more than 10-15 minutes. We treated her based on the results of evaluation during the progress. She filled the VAS and Short Form-36 Health Survey (SF-36) before and after treatment to show the effect of intervention. Three stages of integral therapies were given. In the beginning, hot pack, electrotherapy, and manual therapy were applied during the severe pain period. Stability training was added after 4 weeks. Movement and posture re-education were started after 8 weeks. The full treatment course lasted for 12 weeks with 3-4 treatment sessions per week. VAS was re-assessed every four weeks. Results: VAS decreased from 10 to 3; still-sit time improved to 30-45 minutes; bodily pain, validity, social function, and mental health on the SF-36 Health Survey were improved as compared to the baseline. Conclusion: We suggested this integrated intervention physical therapy for clinical therapy with evidence-based approach effectively relieved CLBP in a taekwondo athlete.
