在葡萄糖攝取與運動壓力下頂尖跆拳道選手血糖恆定變化之研究

Glucose Homeostasis in Elite Taekwando Athletes under Glucose Feeding and Exercise Condition

摘要


本研究的主要目的,在於觀察頂尖跆拳道選手處於葡萄糖攝食與運動等兩種狀態下,血糖恆常濃度的控制能力。頂尖跆拳道選手與對照組(一般體育學院學生)共十二位女性受試者共同參與本研究,這兩組受試者於隔夜禁食後,進行葡萄糖耐受度測試與血液樣本之收集。在75公克葡萄糖攝取後,兩組受試者血液葡萄糖濃度均明顯提升,其中跆拳道選手在葡萄糖攝食後30與60分鐘的血糖之上升與回降濃度,均明顯較對照組的受試者低。在相同的實驗時程上,兩組受試者血液胰島素的濃度起伏與葡萄糖濃度變化等差異也類似。因此本研究發現,頂尖跆拳道選手在碳水化合物補充狀態下,對血液葡萄糖吸收能力,比一般運動員較為卓越,而此差異部份與全身胰島素敏感度有關。在運動壓力測試狀態下,所有受試者在相同配速下,進行15分鐘反複式爬坡跑,在完成運動後立即測量生長激素與血糖濃度,並與運動前(空腹狀態)作比較,來觀察運動所造成的壓力水準。這個運動明顯造成所有受試者生長激素濃度顯著上升,然而在頂尖跆拳道選手生長激素濃度昇高的幅度較對照組為少,顯示頂尖跆拳道選手在相同量的運動刺激下,身體所感受的壓力反應較小。在血糖濃度變化方面,跆拳道選手在運動後平均血糖濃度顯著比對照組高,顯示頂尖跆拳道選手在運動壓力狀態下防止血糖濃度下降的能力較佳。這些特質也許對於每天須應付數場比賽,而能源儲存時間相對短暫、且比賽動作兼具重複性、持續性與強度之不可預測性的運動競技具有功能上的重要性。

The purpose of the present study was to examine the degree of blood glucose fluctuation under glucose feeding and exercise conditions in elite Taekwando athletes. A group of elite Taekwando athletes (female, N=6) and PE students (female, N=6) as a control group carried out OGTT after an overnight fast. Following a 75-gram of glucose challenge, the blood glucose concentration of these elite Taekwando athletes was smaller at 30 and 60 min post than the control group. Insulin concentration followed a similar trend as the glucose fluctuation under this condition. These results indicated the better glucose tolerance in these elite Taekwando athletes is partially due to higher in vivo insulin sensitivity. All three groups of the subjects also performed a 15-min repeated hill running with the same pace. Blood growth hormone and glucose concentrations were measured before and after the exercise in both groups. The growth hormone response to the exercise was significantly greater in control subjects than the Taekwando athletes. Accordingly, these Taekwando athletes had a significantly greater post exercise blood glucose concentration compared to control group. We conclude that the elite Taekwando athletes had a characteristic of better maintenance of glucose homeostasis under feeding and exercise stress, and that trait may be important for them to cope with the chronic and repeated types of sports competition.
