Elderly with Tetralogy of Fallot Complicated By Multiple Brain Abscess-A Case Report

摘要

法洛氏四合症患者，若未接受手術治療，只有少數能夠活到成年期；且未經過開刀嬌正之先天發紺性心臟病是一個容易引起顱內膿瘍的重要危險因子。在此我們描述一位罕見老年人其患有法洛氏四合症未經開刀嬌正，之後併發多發性顱內膿瘍並呈現右側肢體乏力之病例。患者於外科加護病房接受適當之抗生素治療及內視鏡立體定位手術清除顱內膿瘍後，狀況持續改善。膿瘍培養長出鏈球菌。於治療十個星期後再追蹤影像學檢查時顯示顱內膿瘍均已完全消除並且無留下神經學缺損。

In tetralogy of Fallot, only a few patients reach adulthood without surgical repair, and uncorrected cyanotic congenital heart defect is an important predisposing factor for brain abscess. We describe one rare case of elderly with uncorrected tetralogy of Fallot who was complicated by multiple brain abscesses and presented with symptoms of right side limbs weakness. The patient was treated in the surgical intensive care unit with adequate antibiotic injection and stereotactic endoscopic removal of abscess, then, his condition continued to be improved. Abscess culture grew Viridans Streptococcus. Radiological follow up performed ten weeks after treatment demonstrated total resolution of brain abscesses without neurologic deficit.
