In tetralogy of Fallot, only a few patients reach adulthood without surgical repair, and uncorrected cyanotic congenital heart defect is an important predisposing factor for brain abscess. We describe one rare case of elderly with uncorrected tetralogy of Fallot who was complicated by multiple brain abscesses and presented with symptoms of right side limbs weakness. The patient was treated in the surgical intensive care unit with adequate antibiotic injection and stereotactic endoscopic removal of abscess, then, his condition continued to be improved. Abscess culture grew Viridans Streptococcus. Radiological follow up performed ten weeks after treatment demonstrated total resolution of brain abscesses without neurologic deficit.
