聲門下血管瘤(sublottic hemangioma)是一種良性腫瘤,但在嬰兒身上會造成極爲嚴重甚至對生命具威脅性的呼吸道阻塞。往往病人在幾個月大就會有症狀,而且一直被誤以爲是複發性或持續性哮吼,直到放射線影像學檢查、內視鏡檢查、切片檢查後所得的確定診斷已太遲。我們報告一個臨床表現最先呼吸急、喘、吸氣性哮鳴和狗叫聲般咳嗽的聲門下血管瘤。這些症狀和徵候與哮吼相當類似,臨床醫師對此位置産生血管瘤的可能性要有高度警覺心。更特別的是,在此病童小腦發現有類似血管瘤的病竈。病童接受注射與有用類固醇後,左側眼瞼與臉頰的血管瘤已消失,但聲門下血管瘤與小腦類似血管瘤的病竈仍有存在。
Sub glottic hemangioma (SGH) is a benign neoplasm that may cause severe and life-threatening respiratory obstruction in infants. However, patients usually present with inspiratory stridor in the first few months of life and may be mistakenly diagnosed as recurrent or persistent croup. Definitive diagnosis is made by image studies, endoscopic examination and biopsy or all. We report a 2-month-old female infant of SGH with initial clinical manifestations of dyspnea and inspiratory stridor co-existing with cutaneous and cerebellar hemangiomas. Clinicians must be alert the possibility of SGH when associated with cutaneous hemangioma. This patient has received oral steroid treatment for more than two months with improvement of the airway obstruction. Although purplish patch lesions over left side of face, eyelid, cheek, and pen-oral regions regressed, the size of the SGH on the followed MRI was slightly enlarged. The diagnosis and various treatments of SGH are discussed and reviewed in this paper.
