Exogenous Estrogen Stimulates the Increase of Circulating Leptin in Normal Cyclic Women

背景及目的:最近雌激素與瘦體激素被認為可能有相關關係,本研究的主要目的在於探討外因性雌激素,是否會直接影響正常月經婦女瘦體激素之變化。 方法:41位正常月經婦女志願接受此研究。月經週期卵泡期第五天時,21位給予直接靜脈注射20毫克之結合性雌激素,而另外20位則注射生理食鹽水做為對照群。之後,定時的抽血測定及分析兩群之間血中雌激素、瘦體激素、黃體化激素及卵泡刺激素之濃度及變化。 結果:靜脈注射雌激素後4小時,血中雌二醇即呈現有意義的急速上昇,且持續至注射後96小時。而血中瘦體激素亦於注射雌激素後28小時,逐漸形成有意義的上昇,並於32小時達到高峰,然後纔慢慢下降。然而,對照群婦女血中之瘦體激素並無任何上昇之反應。而血中之黃體化激素及卵泡刺激素,於雌激素注射群則呈雙相性之反應,但對照群則沒有任何變化。 結論:此研究結果顯示,卵泡期時高濃度之外因性雌激素可刺激瘦體激素之上昇。此現象反映出雌激素可經由內分泌機轉來調節脂肪細胞產生瘦體激素。

Background and purpose: A possible relationship between estrogen and leptin has recently been documented. The aim of the present study was to examine whether exogenous estrogen affects changes in leptin levels in women with normal menstrual cycles. Methods: Forty-one healthy, normal-weight, normal-cyclic women volunteered for this study. Twenty-one subjects received bolus intravenous injections of 20 mg conjugated estrogens (Premarin group) at 8:00 AM during the fifth cycle day after fasting, and 20 women received 20 mL saline intravenously (control group). Blood samples were collected at 0, 4, 8, 24, 28, 32, 48, 56, 72, and 96 hours after injection. Plasma leptin, estradiol (E2), luteinizing hormone (LH), and follicle-stimulating hormone (FSH) levels were assayed by radioimmunoassay. Results: The mean circulating E2 level significantly increased form 4 ~ 96 hours after Premarin injection. Compared with preinjection levels, mean leptin level significantly increased at 28 hours (P<0.05), peaked at 32 hours (P<0.001) and sustained this peak at 56 hours, and remained significantly elevated at 72 and 96 hours (P<0.05) after injection of estrogen. However, there was no change in plasma leptin levels in the control group. Biphasically secretory patterns of LH and FSH were observed in the Premarin group only. Conclusion: These data suggest that exogenous estrogen can stimulate the increase of plasma leptin concentrations during the follicular phase in normal-cyclic women, and provide evidence that high estrogen levels may influence adipocytes to modulate leptin production through the endocrine mechanism of the reproductive system.
