Metastatic Renal Cell Carcinoma of the Cauda Equina With Hemorrhage and Acute-Onset Cauda Equina Syndrome: A Case Report and Literature Review

Metastasis of tumors to the intradural space is rare, comprising approximately 6% of all spinal metastases. Renal cell carcinoma (RCC) is a highly-metastatic carcinoma, with 25-30% of patients having metastasis at initial presentation. Distal metastasis of RCC to the cauda equina is uncommon. Herein, we reported a 68-year-old man presenting with acute-onset cauda equina syndrome. The final pathologic diagnosis was metastatic RCC of the cauda equina with recent hemorrhage. We reviewed the literature in light of the clinical characteristics of our patient and discussed the possible mechanisms of metastasis. Although RCC metastasizing to the cauda equina is very rare, differential diagnosis should include the possibility of metastatic RCC of the cauda equina in patients with a history of RCC and back pain or weakness of extremities. Surgical treatment for resectable cases is necessary, as the neurological outcome is good.
