照顧一位兒童齒源性蜂窩組織炎之護理經驗
Caring for a Child with Odontogenic Cellulitis
摘要
本篇個案報告主要描述照護因牙齒齲蛀造成顏面部及頸部蜂窩組織炎，行腫塊切除及膿瘍引流術之照護經驗。筆者於97年3月25日至4月07日期間，運用Gordon十一項健康功能評估，經由實際觀察、參與照護及身體評估，將資料加以整理分析，發現個案有疼痛／因術後傷口引起、潛在危險性感染／與術後傷口及管路留置有關、健康維護能力改變／缺乏口腔保健行為、照顧者角色緊張／案母缺乏傷口護理及居家後的照顧訊息等護理問題。在照護過程中，筆者扮演支持者、教導及諮詢者的角色，給予個別化的護理措施，教導有關口腔清潔預防牙齒齲蛀的重要性，加強個案執行健康行為的能力；提供傷口疼痛護理，以減輕個案手術後傷口疼痛；教導照顧者對術後傷口及居家照護方法，促進其日後出院個案的照顧能力。藉此提昇護理服務品質，使個案能獲得連續性與整體性的照護。

In this case report is described the nursing experience caring for a patient with facial and neck cellulites caused by a decayed tooth after it was excised and drained. Data from Gordon's 11 health function, observations, actual care, and physical evaluation were collected from March 25, 2008 to April 7, 2008. The subject had problems changing health care and oral sanitation habits and post-surgical wound pain, and her caregiver had stress because she was not well-informed on wound care and homecare. The nurse provided the patient with personalized nursing care, shared knowledge on the importance of oral hygiene and prevention of cavities, taught her how to better maintain oral health, and sought to relieve wound pain. She educated the caregiver on post-surgical wound care and homecare. Through these efforts, the patient's chances of infection and further complications were reduced and she was able to receive better continuous care.
