目的:旨在探討澳門癌症患者被告知癌症診斷的心路歷程,以了解目前癌症診斷告知的寶踐現況及患者被告知青主斷後的感受。方法:採質性研究設計,以立意取樣選取十名癌症患者,通過深入訪談方法收集癌症患者被告知癌症診斷的心路歷程。結果運用內容分析法從患者經驗歸納及識別出兩大主題:(1)癌症診斷告知的方式(包括告知對象、告知內容)(2)癌症診斷告知後患者的心路歷程(情緒失控、情緒穩定、內心岩、志的折騰、家庭社會的支援、正向思維的產生)。討論與結論:受訪癌症患者都未能詳細了解自己的病情及得到醫療人員關顧自己的感受,因而增加患者的焦慮及無助感,值得檢討臨床現行告知癌症患者診斷的內容深度。得知患癌後患者內心的顧慮及掙扎往往比外在表現來得複雜,而配偶、病友及宗教團體對患者起到重要的支持作用,作為醫護人員應鼓勵及調較這些角色於癌症患者的照護工作中。
Aim: To explore the experiences of diagnosis disclosure of the cancer patients, in order to understand the current practice of cancer diagnosis telling and the feeling of the patients being disclosed. Method: Qualitative approach was adopted by conducting in-depth interview with 10 cancer patients who were purposely selected in a Cancer Therapeutic Center, to collect information on their experiences of being told the diagnosis. Resu1ts: Content Analysis was used to conclude and identify two main themes out of the 10 interviews. They are (1) Way of disclosing cancer diagnosis-includes target and content of disclosure. (2) Experiences of cancer patients after being disclosed the diagnosis-includes emotional disorder, calmness, struggling in the heart, support from family and society and formulation of positive thinking. Discussion and Conclusion: Interviewees experience extra anxiety and helplessness because they had not been told about the details of their disease and had not been given chance to express their feeling. This highlights the necessity of reviewing the way and content of cancer diagnosis telling Cancer patients undergo more complicated struggling and suffering in the heart than how they behave, while can be well relieved by the support of spouse, peer patients and religious groups, This reminds that healthcare professionals can play an important role to encourage and involve these people in caring for cancer patients.
