出生一週新生兒的體重、哺餵方式與黃疸變化的相關性研究

Relationship among Body Weight, Feeding Methods and Jaundice of One Week Old Neonates

摘要


目的:探討新生兒一週內之體重流失率、餵奶方式與黃疸變化的關係。方法:參予研究之新生兒93位,分別以經皮黃疸測定儀,測量11次黃疸指數。以描述性統計、皮爾森積差相關、單因子變異數、簡單回歸等進行分析。結果:出生第四天體重流失量及百分率最高。出生120小時為黃疸為高峰時間。哺餵純母奶者於第二、三、四天,體重流失率最多。哺餵純母奶者於第一、二、五、六、七天,其黃疸指數均較高。第二、三、四、五天體重流失率與120小時黃疸指數,有正相關。第四天體重流失率預測出生後120小時之黃疸指數,解釋力10.4%。結論:醫護人員應持續監測新生兒攝食之奶量與體重流失及新生兒黃疸,並在新生兒出院前,評估產婦哺餵母乳技巧、新生兒能正確含奶及有效吸吮乳房等,讓新生兒獲得足夠奶量。

Physiologic jaundiceis a commonand transient problem for neonates after birth. However, pathologic jaundice is the major cause of readmission of neonates after discharge. Objective: Relationship among the weight loss, feeding patterns, and jaundice of neonates born in the first week Methods: Ninety-three neonates born were enrolled. The transcutaneous bilirubin was measured with a noninvasive Bilicheck at 11 time after birth. Data analysis was performed with Pearson correlations, student t-test, one way analysis of variance, and simple linear regression. Results: Result showed that in the first week of life. Neonatal weight loss reached the maximum on the fourth day. The jaundice level measured reached its peak at 120 hours respectively. The highest percentage of weight loss is in the exclusive breast feeding group on the 2nd, 3rd, and 4th days. The jaundice level were significantly higher on the 1st, 2nd, 5th, 6th, and 7th in the exclusive breast feeding group. Percentages of weight loss on 2nd, 3rd, 4th, and 5th days were significantly correlated with the jaundice level at 120 hours. At 120 after birth, 10.4% of variance of jaundice levels were explained by the percentage of weight loss on the 4th day respectively. Conclusion: Generally, physiologicial jaundice is harmless to newborn, We should keep monitor of level of bilirubin to evaluate jaundice.Physiologic jaundiceis a commonand transient problem for neonates after birth.
