協助一位食道癌術後個案脫離呼吸器之護理經驗

Nursing Experience with an Esophageal Cancer Patient Being Weaned from a Mechanical Ventilator after Surgery

摘要


本文為照顧一位食道癌術後患者脫離呼吸器之護理經驗,筆者於個案住院期間運用身體評估及Gordon十一項健康功能型態評估,藉由觀察、會談及病歷查閱等方式來確立個案之健康問題;包括有呼吸道清除功能失效、輕度呼吸機戒斷反應功能失常、腹瀉、無望感等護理問題。筆者提供個別性之護理措施,減輕病患生理上的不適外,並提供個案及家屬有關疾病進展及呼吸器脫離相關知識,在照顧過程中持續關注個案及家屬的情緒反應,鼓勵表達出心中感受,參與治療及自我照護,順利脫離呼吸器轉出。期望藉此照護經驗提供護理人員日後照護類似個案能儘早發現病患心理問題,及時與醫療團隊共同擬定呼吸器脫離計劃,以協助個案順利脫離呼吸器回歸社區。

The following article discusses the nursing experience in treating a patient with esophageal cancer and assisting him through the process of being weaned off endotracheal intubation. The author performed a thorough general survey of the patient's condition through Gordon's 11 functions of health appraisal as per the appraisal Guide, close observation, various interviews and reviewing the patient’s chart. The patient’s problems included ineffectiveness in clearing the respiratory tract, mild difficulty from recovery from endotracheal tube weaning, episodes of diarrhea, and a sense of hopelessness. The author provided individualized nursing care according to the patient’s needs. The care provided included: alleviating the patient's physical discomfort; and providing the patient and family members with information in regard to the disease progression and basic knowledge of the weaning process. During the course of nursing, special attention was paid to the patient's and family members' emotions and they were encouraged to communicate their feelings and participate in self-care, and this ultimately led to success in weaning of intubation. This article discusses and provides nursing experiences that may prove helpful for other nursing members who may encounter similar cases in the future.
