出院準備服務介入法洛式四重症病童之照護經驗
A Nursing Experience of a Child with Tetralogy of Fallot through Discharge Planning Intervention
摘要
本個案報告是應用出院準備服務過程，協助一位法洛式四重症病童與家庭，共同完成出院準備服務之照護經驗。個案護理時間為95年9月20日至10月11日，照護期間經由觀察、會談、家庭評估、溝通及主動傾聽等收集資料，將照護與互動過程的內容詳細紀錄，以出院準備服務之概念，針對內容加以分析確立個案的護理問題包括：主要照顧者之健康尋求行為、營養不均衡：少於身體所需及家庭治療計畫處置不當等。在護理過程中，提供主要照顧者有關個案居家照護之胸腔物理治療，共同討論爲偏食個案設計多樣且足夠熱量之一日飲食計畫表，提供家庭環境的調整與心理支持，提升主要照顧者照護先天性心臟病病童的能力，以減少因長期照護壓力對整個家庭帶來的影響，使個案得到最舒適且持續性照護，希望提昇主要照顧者對個案居家照護正確知識，進而降低個案疾病的合併症發生及減少再入院的機會。

This case study is a nursing experience of the discharge planning procedure about a child with Tetralogy of Fallot (TOF) and his family. The subject was admitted from 20(superscript th) September to 11(superscript th) October 2006 and data were collected throughout this period by observation, interview and family assessment, active listening, etc. The nursing problems of the subject included: 1. Health-seeking behaviors (specified) of the caregiver; 2. Imbalanced Nutrition: Less Than Body Requirements; and 3. Ineffective family therapeutic regimen management. Regarding the nursing period, the primary caregiver had received many individual nursing education skills on home care. These included chest physiological therapy, CO-design of a daily diet of rich nutrition and calories, mental support of primary caregiver, nursing goals for improved caregiving to take care of the CHD child in home care, and to decrease the impact of long-term care on the family. The study aims to decrease the occurrence of complications and chances for re-admission through enhancing correct health care knowledge of the primary caregiver.
