摘要

本文為敘述一位中年婦女因腦腫瘤術後合併右側肢體偏癱，導致個案面對疾病所引發身體功能喪失，產生身體與心理問題之護理經驗。藉由整體性健康評估，以觀察、會談、身體評估等方式收集資料，分析歸納個案主要健康問題：腦組織灌流改變、身體活動功能障礙、無力感及家庭運作過程改變。依據個案需求，利用醫療團隊資源，與個案及家屬共同擬訂復健計畫，鼓勵個案積極參與肢體活動功能訓練。幫助個案獲得正向支持，能調適心理，度過無力感危機。協助家屬建立互動式照顧脈絡，提供輔具租借中心、喘息服務資源。期待個案在出院後，家屬有足夠能力照顧個案。

This report describes a middle-aged woman with a tumor of the brain. The patient suffered from postoperative hemiplegia, encountering physical and psychological problems as a result. The information was obtained by interview and observation, finding that there were four major problems. (1) altered cerebral tissue perfusion; (2) impaired physical mobility; (3) powerlessness; and (4) altered family processes. According to the patient's needs, the author worked with the medical team and drafted rehabilitation plans with the patient and her family. They were encouraged to participate actively in physical activities and functional training. Nurses provided positive support for strengthening of psychological adjustment to overcome the crisis of powerlessness; furthermore, assistance was given to the family in looking forward to creating interactive care as well as providing rental devices and respite care services. We expect that the family would have enough capacity to take care of the patient after discharge.
