Nursing Experience for the Care of an Elderly Patient with Total Hip Joint Replacement Surgery

摘要

本文敘述老年患者因股骨頸骨折接受全髖關節置換術之護理經驗，2015年12月18日至28日，運用Gordon十一項功能性健康型態評估，確立個案有急性疼痛、身體活動功能障礙、自我照顧能力缺失及焦慮。照護期間，介入疼痛緩解處置，減輕傷口及移位產生的疼痛，同時連結醫療團隊，與個案共同訂定個別性復健計畫，改善身體活動功能障礙，提昇自我照顧能力，同時傾聽個案感受，引導說出內心想法，並利用出院準備服務概念，協同物理治療師共同規劃居家環境及指導居家照護技能，協助減輕個案擔憂返家照護引起的焦慮。藉此經驗分享，做為此類病人照護參考。

This article discusses a case of an elderly patient with femoral neck fracture and total hip joint replacement surgery. During the period from 18th December to 28th December, 2015, by applying Gordon's 11 functional Health Assessment, it was determined that the health problems related to acute pain, physical activity dysfunction, self-care deficit and anxiety. During the nursing period, we provided pain relief treatment to reduce the patient's pain caused by the wound or movement. In the meantime, we communicated with the medical team to establish an individual rehabilitation plan to improve physical activity functioning and to enhance self-care ability. By listening to the patient, we guided the patient to express his inner thoughts and we took advantage of the concept of discharge planning to invite a physical therapist to educate the patient about the safety of home environment and home care skills for decreasing his anxiety for returning home. We hope through this article to share our nursing experience as a reference for the care of an elderly patient with total hip joint replacement.
