運用歐倫（Orem）自我照顧理論照護一位米勒費雪症候群病患的護理經驗

摘要

本文是探討一位20歲罹患米勒費雪症候群男性，因神經病變及對疾病治療相關知識缺失導致日常生活無法自我照顧。筆者於2018年9月12日至9月19日護理期間，運用歐倫（Orem）自我照顧理論，藉由觀察、會談、病歷查閱和身體評估進行資料收集，確立個案護理健康問題有自我照顧能力缺失、身體活動功能障礙、疾病及免疫球蛋白治療相關知識缺失、低效性呼吸型態。依歐倫理論自我照顧之完全代償、部份代償、支持教育性護理系統提供護理措施。照護內容包括提供個別性支持性護理，共同擬定復健目標，用討論增加個案的自我照顧和身體活動能力與認知學習。期望藉由此護理經驗分享，提供護理同仁照護相關個案之參考。

The purpose of this case study was to describe a nursing experience with the application of Orem's self-care theory to care for a patient with Miller Fisher syndrome in the acute phase. The nursing period was from September 12^(th) to 19^(th), 2018, and the author collected data by observation, face-to-face interview, medical record review, and physical examination. The patient's nursing problems included impaired physical mobility, self-care deficit, ineffective breathing pattern and deficient knowledge. According to Orem's nursing system, the author provided nursing interventions including the wholly compensatory system, the partially compensatory system and supportive education system. The nursing interventions provided individual support to communicate with the patient about setting rehabilitation goals and improving self-care ability, physical ability and cognitive learning by discussion. This study presents this nursing experience so as to provide a reference in future clinical nursing care.
