某加護病房醫材登記之持續性品質改善方案

摘要


全民健保實施以來,各大醫院莫不將開源節流列為重要政策方針,比方案即在改善某加護病房之醫材漏帳率,以達病房及醫院的收支平衡;經問卷調查結果發現:課程的提供不足以致不熟悉醫材登記單內容、登記單的擺放位置不明顯、登記單欄位不全、認為記帳是書記的職責、認為記帳與個人績效無關…等因素導致漏帳率達20.02%,故於1999年5月15日至8月15日開始執行本方案,建立醫材登記標準流程及品管稽核,實施後漏帳率由20.02%降為3.48%,顯示改善方案確有成效,並顯著減少病房及醫院的成本流失。

Since National Health Insurance had been implemented, many hospitals have advocated a policy of economizing on expenses. The objective of this project was to decrease the rate of missing records of charges for hems used by patients in the intensive care unit to minimize medical expenses. A total of 272 items of medical materials were surveyed from May 15 to May 30, 1999. The rate of missing charges was 20.02%. The reasons for not recording charges included nurses not familiar with content of charge list, location of charges list not known, charge list incompletely filled out, nurses thought charging for materials was the duty of a clerk, charging for hems not related to personal achievements. In order to improve these situations, the project was conducted from May 15 to Aug 15, 1999. A standard to record medical charges and a plan of quality assurance were established. After this project, the rate of missing charges decreased from 20.02% to 3.83%, which indicated that the project was effective in reducing hospital costs.
