提昇某加護病房家屬執行居家照護技能完整之方案

神經科的疾病屬長期慢性病，常伴隨某種程度的功能喪失，病患出院後照顧多半在家屬身上，故指導家屬學會居家照護技能就顯得非常重要，如此病患離開醫院後，仍能獲得妥適的照護。本病房定期舉辦以居家照護為主題之團體衛教，並配合個案需求給予家屬個別指導，結果發現家屬執行居家照護技能之完整性偏低。因此，本病房成立專案小組以提昇家屬執行居家照護技能完整性。藉由建立家屬照護技能作業流程、照護指導單張、完整指導內容並讓家屬實際操作後，加以評值其完整性。推行結果，家屬照護技能完整性由20%提昇至96%，達專案目標。本專案解決問題的方法不僅改善護理指導品質，更能有效提昇家屬居家照護能力，故值得在臨床上繼續推展。

Neurological diseases are long-term illnesses with high-degree dysfunction on patients. The patient's family is assuming the most responsibilities to take good care of patients. Therefore, educating patients' family at hospitalization with good and safe care-skills is becoming very essential. An example unit regularly took place health education for families, and met patients' needs with individual instruction. However, the result discovered lower complete skills on home-care by patients' family. To improve the complete home-care skills by the patient's family, the demonstration unit with team-work produced such day routine as home care, and the brochure of health instruction. After the patient's family completed practices, the unit evaluated the completely. Final result achieved the goal of this project, the completely increased from 20% to 96%. The project not only solved problems, and enhanced the quality of nursing, but it also advanced skills of home care. The project will be a good example of taking care of patients at home.
